How Does Online EMDR Therapy Work?

If you've been considering EMDR therapy but aren't sure how it translates to an online setting, you're not alone. It's one of the most common questions I hear from people who are curious about EMDR but hesitant about working via video. The good news is that online EMDR is not only possible — research consistently shows it works just as effectively as face-to-face therapy.

Here's a straightforward guide to how it works.

What is EMDR therapy?

EMDR (Eye Movement Desensitisation and Reprocessing) is an evidence-based therapy developed to help people process and heal from trauma, distressing memories, and overwhelming experiences. It is recommended by the National Institute for Health and Care Excellence (NICE) and the World Health Organization as a first-line treatment for PTSD.

Unlike traditional talking therapy, EMDR doesn't require you to describe traumatic events in detail. Instead, it works with the brain's natural ability to process and store memories — helping experiences that have become "stuck" to be filed away in a new, less distressing way.

How does EMDR work online?

Online EMDR follows exactly the same structured, eight-phase protocol as in-person therapy. The key difference is that sessions take place via a secure, encrypted video platform rather than in a therapy room.

The eight phases are:

  1. History taking and treatment planning
  2. Preparation and building resources
  3. Assessment of target memories
  4. Desensitisation using bilateral stimulation
  5. Installation of positive beliefs
  6. Body scan
  7. Closure
  8. Re-evaluation

Each phase builds on the last, and the process always moves at a pace that feels safe and manageable for you.

What is bilateral stimulation and how does it work online?

Bilateral stimulation is the core mechanism of EMDR. In a traditional in-person session, this is often delivered through eye movements — a therapist moving their fingers side to side while you track them visually. Online, bilateral stimulation is delivered in a number of ways:

  • Visual tracking via a dedicated online tool that displays a moving dot on your screen
  • Self-tapping — alternately tapping your knees, shoulders, or hands, guided by your therapist
  • Audio tones — alternating sounds delivered through headphones

All of these are equally effective. Many clients are surprised by how natural bilateral stimulation feels once they get started, regardless of whether it's delivered in person or online.

What do I need for online EMDR?

Very little. You'll need:

  • A laptop or tablet with a camera and microphone
  • A stable internet connection
  • A private space where you won't be interrupted
  • Headphones (recommended but not essential)

Is online EMDR as effective as in-person EMDR?

Yes. A growing body of research confirms that online EMDR produces outcomes equivalent to face-to-face therapy. A significant number of my clients actually find the online setting preferable — processing difficult experiences from the familiarity and comfort of their own home can feel safer and more contained than travelling to a therapy room.

Who is online EMDR suitable for?

Online EMDR is suitable for adults experiencing the effects of trauma — whether recent or from many years ago. It can be particularly helpful if:

  • You've tried talking therapy but felt it didn't quite reach the root of things
  • You have a busy schedule and find it difficult to attend in-person appointments
  • You live outside London or abroad and want access to an accredited EMDR practitioner
  • You simply feel more comfortable processing difficult experiences from your own home
